With the passing of the Therapeutic Shoe Bill in 1993, Medicare (part B) extended coverage to diabetic patients for footwear and foot inserts. Protective footwear can help in the fight to prevent the thousands of diabetic amputations that occur each year.
What is covered? Coverage is typically limited to one of the following within 1 calendar year:
· 1 pair of standard therapeutic shoes and up to 3 additional pairs of custom moldable inserts. (Note: typically 1 pair of night support stockings per every 5 years can be approved). · Customized shoes and inserts may be available on a case by case basis if resident has partial amputation or other diabetic-related foot complications.
Coverage is limited to shoes and inserts meeting certain requirements for patients with specific clinical conditions, including: · Diagnosis of diabetes and presence of a comprehensive care plan for diabetes by their physician. · Past amputation or foot complication. · Presence of peripheral neuropathy. · Physician approves diabetic shoes as part of resident’s comprehensive care plan for diabetes.
Consult with your CMS clinical representative for more information. Some key things to keep in mind are:
· Medicare pays 80%; resident is responsible for remaining 20% co-pay. · Resident must be under a part B stay in order to receive therapeutic shoe benefits from Medicare. To check an individual resident’s benefits, call 1-800-Medicare. Be prepared to provide resident’s Medicare ID number and information about their medical status |

Research studies have demonstrated that diabetic footwear (as part of an overall plan of care) can have a positive impact in preventing diabetic related foot complications. Because of this, in 1993 Medicare decided to begin covering diabetic footwear for qualifying elderly residents. |
